OK I can tell you what your problem is.
You play single target nukes. And in ELO hell, playing a mid laner single target nuke is generally bad, because people are stupid and dont know how to teamfight. If you want to stay mid, you either need to go to a more utility based champ that makes teamfights a no brainer (like
Veigar) or go for massive AOE damage (like
Brand,
Swain,
Kennen, or
Zyra) so that you can crush entire teams at once.
Remember. People are stupid. You have to carry by destroying entire teams at once.
LeBlanc cant do that.

Fruyti Kitty mains Leblanc and is 2k ELO. The problem is Leblanc carries super hard and is super scary but if you let the game run too long she becomes a lot less useful. The others have similar problems. You need to remind your team to win early if you're playing Leblanc or make sure your team gets other champs that scale well into late game like
Vladimir and
Tristana.
If you're playing single target assassins you increase your usefulness by warding around and catching people off-guard. If you ward their jungle and see the enemy AD Carry running around in it you can 100 to 0 them and run out, and you just gave your team a huge advantage. Even if you suicide into their carry and kill them you have done your job, because late game they are much more valuable than you. You also can't go as crazy diving into the whole team anymore, and have to be more tactical about your positioning / when you decide to go in.

Play jungle if you want to carry your team. Help every lane by ganking them at least once to give them the advantage. I raised my ELO from 1500 to 1800 within two days just by playing maokai, skarner, and nautilus; ALL the same build core build, masteries, and runes.
If i want to get more ELO, i jungle maokai. All there is to it.

Since I fell down in ELO, I decided to take the time to rework on myself and determine my weakness, which, I fount out, is that I'm completely unable to carry games.
My favorite champs are AP Assassins :
The following situation actually happens quite a lot when I'm playing : I do a good early/mid game, I get a nice ambiance by providing ganks, warding for others sometimes, counter ganking and giving tips, which causes me to become some kind of "counselor" of the team with people asking me advice about their build and the strategy to follow in team fights, but then late game happens and I just don't seem to get the right strategy to carry the teams to victory. Makes me feel really bad when that happens as I end up betraying people's expectations.
I hope to get some advices on how to avoid making bad calls and on how to use the teams cohesion and that position I'm sometimes given to carry my team to victory.
