No advice to give. Just that I know that feel. I feel like I die too much now. My friends and I used to laugh about how I never died (like ever) in my games even playing as squishies (hence the reference in my sig)... But now I die like 5 times a game. It feels wrong dude. I feel like I'm feeding when I've got 2 deaths, so five is like... bad.
I remember back when I played ADC exclusivly with tristana and vayne, I was like level 20. I always ended up with like 25/2/0 or some crazy stuff with more than half the team kills. now that My Que for normal is getting higher I pull this off less and less, I feel like I'm bad, but in reality its just that everyone else is on my level now. Not sure that's your problem, but it might be. If its not I say just keep playing, you'll get back into it one day with some awesome game and it'll be revival.

I know that feel bro. It sucks. When I hit a hard rut, I do a few things.
1) I hop onto Skyrim, and go murder the ever loving **** out of every single man woman and child in some random city (Usually Windhelm... Racist *******s be hatin on my wood elf). Really a good de stressor.
2) I read a new (decent) guide on the character I just lost as. Sometimes someone makes a decent point about something you didn't ever think about before (even if the rest of the guide was rather ****, sometimes there are gems of chocolate filled goodness nestled in a pile of poo).
3) I read a high rated guide on the champion that beat me (because more than likely that's what the person was following that beat you). In the decent guides there are not only "how to be a boss" sections, but there are also "uh-oh situation" sections. Pay attention to those, it will tell you how to beat that champ according to a popular guide that alot of people probably follow.
4) All in all, just try to remember that this is a game, a way to relax at the end of the day and pass time by having fun. If you get really stressed out over it, you ruin the point of the game, and it starts to become a job. Chill out, ELO is like the stock exchange. It goes up, it goes down, all part of the cycle. Unless you're Reginald. That guy is like Google stock, straight up.

Hey, if you liked any advice i gave, be sure to +rep me! Oh, and check out my Shaco and Maokail lorecraft in my Blog! I'm a starving artist in need of attention, so please feel free to read, respond, and critique me! Please? Anybody that leaves a comment (good or bad) will receive a +rep!
Don't worry about it. Clearly you're playing on a tilt and you're not going to be at your peak until you come back down and get that relaxed "in-the-zone" feeling again. I recommend doing some arranged 5v5 in normals just to get some coordination and seriousness going while still having fun and focusing on what you're doing.
If all else fails, break for a little bit until that flustered feeling wears off instead of just plowing through it. Either way.

2 weeks back, 1600 elo, climbing 20'ish elo/day. Consecutively carrying games and more often than not winning them, even winning a 4v5. Then i hit 1642 elo, and i got feeders - soo many feeders, ragers, afk in base'ers and the like.
I let the thing i told me would never happen happen. I let solo quee get to me. I got the "Can't Be Arsed" attitude. That's where it hit me. I began playing worse, not just a little worse, a lot worse. Every game i was the baddie, every game i was the feeder that people were reporting.
So i started playing normals just to get back on my feet. I couldn't even keep up with normals and soon i found myself loosing to 1300 elo bot-lanes, just because i'm that bad right now.
10 minutes ago i was duo-queeing with a guy(1700 elo on a 1100 smurf). I told him you don't want to play with me, he insisted and told me np, he'd carry.
We got into picks and i counterpicked their jayce with kennen. A pretty easy matchup. Rengar got FB on me(obvious since their red jungle was at top lane, but yeah that's just me being stupid). I ended the lane around 20 minutes with 0/4/1 and 90 cs. Against jayce(a presumably counterpicked lane)
Right now i'm frequently seeing myself with 100 cs at 20 minutes, which compared to 2 weeks ago(160'ish@20 minutes) is just terrible.
Anyone who has any advice to get back up there?
