I'm on NA, so I won't be able to add you in-game. Still, I have a few suggestions.
First, if you're not already, join the in-game MOBAFire chat room. I'm not sure how active it is on EUW, but you should be able to use it to get into games with some other MOBAFire members. Sometimes.
Second, you might want to install a program to record your games. Then you'll be able to look over your losses (and wins) to try to see what mistakes you made and why you made them. That should help you avoid those mistakes in the future. You can also put the replay files online and have people here download them to look at your games.
Third, join some of the MOBAFire inhouses. The constructive criticism inhouses will have other players who can let you know if you're doing things wrong, and usually we use the MOBAFire Vent so that teams can talk to each other during the games, which will help give you immediate feedback and suggestions.

Basically, I have Leona's mechanics down and I'm already decent with her. Most matches I use her we win. My only problem however is I lack any sort of discipline when it comes to initiating. I tend to jump the gun in team fights whether my team is ready or not and it often gets me killed.
Don't get me wrong I don't run off and zenith blade into the enemy team. It's just that when we are in a stand off or bump into the enemy team, my team always seem to not "get it" when I initiate. I wonder if its me going about it wrong or if they are just not reacting quick enough or not sure what to do when I force a fight.
I guess I would just like to duo with an ADC (who uses Leona) that can give me advice based on my actions.

it's hard to be an iniciator and not have the team on the same page as you, mainly if you are behind.
If behind, avoid.
IF ahead, make sure the team is all on the same page. Maybe ping on a prefrencial target. I usually see myself calling the shots since the team starts to get cocky after a few teamfights and wanders off.

I recently picked up Leona to find that I feel right at home with her natural play style (Tank/support). It also turned out that I have a natural talent for the tank role the first time I used her (2/3/21.) I am a very good team player (Got the yellow leader badge if its worth anything to you?)I'm quite vocal during a match, attentive to what my team is doing and will always help at the first sign of trouble. I look out for my team mates and I'm not afraid to charge the enemy to engage, allow my allies to escape or re-position themselves.
I would really like to be able to take my use of Leona to a pro level due to how much enjoyment I get out of using her. There is nothing quite like passion and knowledge combined after all!
