I've always found it difficult to use item active abilities in the middle of a fight, is there a specific hotkey I'm missing here?

If you can make one, how to do so? I would probably get Active Items more if I didn't feel gimped trying to get to them mid-fight.

The standard way to use them is by using the number keys above qwer but you could also bind new keys to use active items like zxcvbn if you wanted to (just an example, of course).

Yes you can activate them using the numbers above qwer like B-Wong said. They are in order of your items. For example, you activate the item in the first spot of your items by pressing one. I find it best to move the active item to the spot you find easiest to activate.
Haikus are easy

I set up the #1 slot to be assigned to my side mouse button, then I just move my activate-able item into the 1st slot.

I hotkey to T. Some people use the key for smart/self cast, but I usually use it for item actives.
