Regarding your response. Playing 'super-aggressive' is not an excuse. You can play very aggressive without dying a lot. Between 6 and 7 times on average, over like 300 games, is waaay too much and I think it's part of why you (sometimes) lose games. If you end the game with 21/8 that means you've given them at least 2 or 3 major shutdowns with a couple of assists so you aren't really helping your team that much then.
Just my advice. Do with it whatever you want.
Yeah I know I die too much. It should probably go down now that I don't use Tiamat though, and aside from that, every death is a situation I played wrong which I can learn from. If I died 6.odd times in season 3, I should die less this season. :P Right now is not a very good time to test it though seeing as I seem to be on the losing team pretty much every game, and it's a lot harder to die less if I'm losing every team-fight as opposed to winning every team-fight.

i can't really help you with when to go in, but shouldn't you set your team ahead by roaming when you are fed/have enough damage to kill the botlane/toplane?

When you are playing mobile assassins just start roamiing when you are ~20 farm and ~2 kills ahead of your opponent. The time it takes to burn enemy's top lane's

top ganking explanation also applies to bot but usually you earn 2x

If YOU kill 2-3 of them, then you should be winning team-fights and be able to take an objective after each one. Any time you win a team-fight, you need to think of what you can do to either set your team ahead, or set the enemy team behind. E.G., go for drag, a tower, baron, whatever you think you can do. If you can't do any of those, try to take the enemy's buffs. If it's a buff you don't need, tell your team-mates to take it, if they're alive.
You learn the right time to come in with experience. You should of course try to wait for any major CC to be used ( if you're against Amumu, never ever EVER go in as Kha or Talon before his ult has been used ).
