This build is intended for someone wanting to be an anti-carry. When the teamfights start, this Mundo runs right past Rammus and chops up Ashe and Twisted Fate.
Items will give you insane amounts of health, as well as making you someone who can take down almost anyone in a 1v1. Your health regen with your ult will keep you very alive and by end game health wont even be an issue for you. The speed and damage you do isn't maxed, but plenty to drop a squishy in a few seconds while surviving a lot of damage in the process.
Boots of Mobility can be replaced with Mercury Treads and the Spirit Visage can be replaced with a Force of Nature if the other team is really hitting hard end game. But the Spirit Visage is very helpful mid and early late game because it synergizes well with your ult.

I'm sure you know how to lane effectively with Mundo, so I wont waste your time.
Team fights are different however. You are not the tank even though you are very tanky. Do not initiate, but instead stay at the front and throw cleavers at the high damage squishies or tanks that are threatening to initiate your unprepared team.
After your tank initiates, run as fast as you can towards Ashe or Annie in the back and activate all your skills. If you're at full health, wait a second or two before activating your ult if no one is trying to focus you. Activate W and E right as you reach the center of the group and do some damage as you pass through.
If you do this right, their carry (or carries, Mundo can sometimes solo multiple carries at once) will run for high water and out of the team fight if you can chase them far enough. You wont always kill the carry instantly, but you will keep them away from the fight. But if your team can't handle a fight without the enemy's carry, you're screwed anyways.
As Mundo, it's your job to push the envelope in team fights and be slowing high priority targets at all times. Remember Mundo's ABC's of success: "Always Be Cleaving"
