Items

Items vary pretty much, this is just a solid build that may work, there are too many useful items to list them all. A big bonus for your ult and for the team is the Abyssal Scepter for example.

Skill Sequence

You want to max your Q first because you need the Cooldown Reduction to stack it faster, you need to max your E as fast as possible after that because it grants HUGE Armor Penetration, have an AD in your team (like Wukong or Kha'Zix) that can use it. The slow maxing is important against AD carries like Tryndamere.

Spells

From my experience it's better to take Exhaust than Smite. Exhaust is a must have in 3v3, it's even viable to take 2 Exhausts and one Ignite, the Smite doesn't speed up jungling much, it's just to secure the Vilemaw kill.

Creeping / Jungling

E speeds up jungling more than Q, it's not worth taking Q first for stacks since you reach level 2 in the second camp.
Try to do 4 or 5 camps before you can capture the altar. Help your lanes when enemies are pushing, Ghost in when lanes are trading with low HP.
Solo Vilemaw with your ult at level 8 (yes that works).

Getting invaded

When you're getting invaded by the enemy team and they try to steal your altar, try to teamfight them with your team. Unfortunately it is sometimes impossible to win these teamfights, that's why you have to take a very strong early game comp in 3v3s (Cho'Gath/Nasus/Diana are really good picks for teamfighting early).
