Rushing a straight Madred's is optional. I suggest only rushing it when the enemy team is primarily composed of very tanky and heavy characters. If they have a lot of squishies, it's not worth getting.
If you're going to get boots first, you better as hell get BoM to gank early on. If you don't get BoM, you're saying you get 4080G with only Tier 1 Boots. That's a lot of farming, and your ganks will be extremely slow.
You don't have any real defensive items. I suggest Banshee's since you're quite squishy and because you're a carry. INCOMING STUNS! Frozen Mallet will NOT save you.
Heal on Twitch is an awful choice. It'll save you once or twice, but once you hit mid-late game, Heal will not save you because it only recovers a little health, and the fact that you're squishy as hell dosen't help either. Ignite, Exhaust, Flash, Cleanse and POSSIBLY Ghost. You primarily want those 5 spells. Mix and Match.
Your runes, you'll attack fast early game, but you'll just plateau mid game unless you get fed. Get real runes like Desolation Reds, and some Warding blues. Don't just go full AS
true...
that's TRUE damage over time.

Phantom Dancer (2895 gold) is actually more expensive than Madred's Bloodrazor (2750) gold unless the pric3es have been changed, I'm not in game to check it atm.
I see some people have luck stacking daggers on Ashe and Vayne, but haven't tried it on Twitch. As far as Madred's as a last item, my main reason for being against that, is I feel I get so much more out of the item the earlier I get it. Sure the 4% isn't taking off as much health per hit, but health is more valuable earlier in the game when your opponent has less, than it is later in the game when they have excess. That extra loss of valuable health leads to more kills in my opinion, and allows for the enemy team to be pinned down more consistently. So long as you're not screwed over by the rng match making system, giving your team that kind of advantage (through successful gank kills or just making their lane opponents go b) usually translates to a win.
