NO Defensive items in build!?!? This why...
Firstly build attack speed and critical strike while playing on Nocturne. Attack damage is not required at beginning of the game becouse duskbringer raises Nocturnes attack damage while casted and few seconds after it.
After them is time for lifesteal. Becouse of your attack speed you gain much health while farming or jungling so you can line longer and collect money for Bloodthirster. After got it start building for second one - anymore doesnt matter do you take Sceptre or B.F. Sword first.
Stack your Bloodthirsters and start building Infinity Edge. I usyally start on Cloak of Agility to gain more crit change but how the actyal game shows witch is best one.
Last item is The Black Cleaver - mostly becouse it gains some needed attack speed and increases damage. Also it's passive is great!

Starting with Unspeakable Horror gives you good changes for first blood. Fast buy dagger and couple heal potions and rush to push. Wait that someone comes and first fear him and after use exhaust spell to make him unavailable to run - after use ignite if the enemy doesn't seem like to know how to die.
Secondly level up your Duskbringer as much as you can - givinig you attack damage (max +55) when you cast it. It is great for farming and also in fight. Also do harrasment enemy players with Duskbringer but keep your mana on eye.
Paranoia. After got the ultimate your changes are unlimited! Paranoia is best when ganking but it also can be used to escape or saving a friend as it removes all visibility from enemies. At level 6 I recommend to gank the middle line - theres always a free kill for Nocturne. Level Paranoia always when it is possible to increase its range and damage dealt.
Shroud of Darkness is not so effective at beginning of the game - becouse it takes lot of mana and also even if you block an ability it doesn't give so much attack speed. But anyways level Teh Bubble earlier as the guide says if enemy team has Karthus (always funny when you make Karthus cry when he thoughs he gets you down with he's ultimate)or if you feel like you rather buy mana potions and use Shroud than take some hits from enemy harrasmenting. Anyways I don't recommend levelling Shroud of Darkness more than 1 level before every else spells are fully levelled.

Ignite is good for turret diving - Paranoia in Duskbringer - one hit - ignite and get helloutta there. Also if you are chased you can weak your enemy with ignite and Unspeakable Horror and after strike him with Paranoia and you got good changes to kill him.
Exhaust. I though very long witch one to but in my guide, Flash or Exhaust. I use more Flash but exhaust makes enemys unavailable to get away from Nocturne so I put it into the guide. Becouse of Nocturnes fear and Exhaust spell you catch up anyone.
Runes:
Critical change and cooldown reduction runes are great for Nocturne. Also you can use attack speed runes if you like to. I heavily recommend Crit change and ability cooldowns
