I've said most of what I wanted to regarding the build, although I would add alternatives to the Items chapter. Things like Talisman of Ascension, Banshee's Veil, Frozen Heart... y'know? As for runes, I recommend AD marks, HP seals, MR glyphs and armour quints.
1. I think ignite is an okay spell, it's good to use when playing a champion with an aggressive play-style, such as Thresh, however it's not a better option than exhaust in most cases. I believe that your explanation of exhaust, in the items chapter, is incorrect. Exhaust is not a passive spell, it has both a defensive and offensive purpose. Defensively use this spell for peel in teamfights and during ganks, offensively use it for the AS and MS slow while fighting or for the damage reduction during dives and 2v2 situations. Slow can come in handy to setup for ganks. Additionally exhaust's range is about your AA range, you can more or less pretty much walk up to the enemy ADC and exhaust him and he won't see it coming. This sets up for an easy Flay, Q or ult and can be used to engage easily when receiving a gank.
2. Unless you are behind or getting camped, you should rush Targon's Brace over Sightstone because the stack rate goes up dramatically. With only Relic shield you'll only have enough stacks to kill one cannon minion every time it shows up (3 waves) and no other minion. You should explain the item path better in the cheat sheet and in the items chapter. Preferably here's what it looks like: Relic shield -> 1300g Sightstone + Targons -> ~900g Targons + Ruby Crystal -> <900 Targons and consumables.
3. Mikael's is a very meager option for Thresh. Although his abilities can lead you to being oom, he isn't considered to be a mana hungry support as he relies on his auto-attacks to poke. You only buy this item if your ADC absolutely needs it. This isn't when you're against a magic team, but rather when you're against a heavy CC team. You rush it vs heavy CC, not vs AP.
4. You should add your rune and mastery choices to the cheat sheet, not just the guide. Although looking it at it now, you have no runes in your guide either. So, eh, maybe take care of that? Also add your skill sequence in the cheat sheet.
5. Masteries are all right but there are better setups, or at the very least you should include these alternatives: 09 21 00, 00 21 09, and maybe even 00 09 21.
6. You've got the basics of coding down, but there's a lot you could do make this guide better looking. I could tell you what, but it'd be a lot to write in a comment if you're not even interested in any feedback. Message me if you are, I'd be glad to help out.
7. Also I'd turn on "comment to vote" to avoid "troll"-votes (people downvoting your guide without having to comment a reason).

Madlife maxes Q first (E on level 1 and in some situations E like 2/3 times before maxing Q, in heavy trading lanes you can even max E) !
