Twisted fate is one of the dirtiest pushers in the game, and debatably the best. I'll keep this short and tidy.

Early Game
Take mid. Definitely take mid. Try to farm as much as you can by pulling out red cards at appropriate times, and throwing wild cards to get kills and harass at the same time. Your Stacked Deck ability will be pretty strong against champs early game. If you've harassed your opponent far enough, check your stacks, pull a stun, and try to kill mid. No real pressure to though. The moment you hit level 6, look to gank the side lanes with your Destiny.

Mid Game
So, hopefully your team is competent. Mid game is sort of boring. For teamfights, pull out stuns and interrupt bigtime ults, and pick off the squishy carries. Teamfights are definitely not this guy's forte, so try to pressure towers to give your team an advantage in teamfights, should your opponents miscommunicate.

Late Game
This is where you better excel. Cooldown reduction from Nashtor's, your masteries, and runes should allow you to easily manuever from lane to lane pushing. Yes, you will often leave your team to have 4v5 team fights; they will need to keep on applying pressure to the other team while not dying, allowing you to push the other two turrets. People will start to hate you, and your team will hopefully be able to capitalize on the other team's errors.

Item Notes
Note that there is no critical damage involved in this build, simply because turrets... don't get crit'd. Black cleaver should help you take down tanks like Rammus and Shen if you happen to end up in a fight. Your Bloodthirster and Gunblade should allow you to solo many other AD carries late(mainly Tristana, Ashe, MF), especially if you're good at pulling out stun cards.
