Well, if you already got all 5 first items, comparing Ancient's will to the second Rylai makes :

- 30 More AP for Rylai
- 15% Life steal and +30 AP for allies with Ancients'

If you already have a 20 Stack Mejai, Zhonya, Rylai, Abyssal or Void, you really don't care about 30 AP less or more, while the healing can be a very good option.

An other very interesting choice could be a Lich Bane. 80 AP, a little MR, and that ****ing awesome proc. You only lose 20 AP from a Rylai but you have a +500 damage on hit, which is very interesting as you are ranged, snare with the first Rylai and have a ridiculous 3 sec based cooldown on Tranfusion.

When i talked about more AP, i should have say more "effective AP" (lich bane's proc, magic penetration, ...) : if you consider what i said before, you only have abyssal scepter OR void staff so you can go with the second.

Another thing : if you get Mobility, drop the teleport for an ignite or a flash. I usually get Flash/Ignite so i can get easy kills (particularly on lane, often middle). Flash -> Transfusion -> Ignite -> Ulti -> Sanguine pool -> Transfusion can kill barely anything. Usually with alittle harass you don't need ignite (you might even only need transf and ulti), but it's mandatory if there's a ****ing Soraka or summoner heal. Fighting a full life hero, don't forget to stack your tides of blood before flashing and use it while igniting as it doesn't need a target :)

you should go Rylai first (or Mejai if you feel dominating at the beginning). A second Rylai is also a bad idea, you already have health from AP and snares doesn't stack so you could go to something with more AP. Think about tranforming your boots to Mercury or Mobility too depending on other team and what you wanna do (for ganking, Mobility are better).

Rushing Zhonya is rarely a good thing considering its price and there's no progressive way to it.

Usually, i go Mobility/Sorcerer/Mercury, Mejai, Rylai (snare is not an option with Vlad), Ancients' will, Zhonya. If game lasts more, you can go void staff/abyssal scepter/banshee/Guardian angel/Thornmail. You can of course make Zhonya before Ancients', and even replace this last one to get abyssal scepter and void staff, but i really apreciate the bonus healing provided by Ancients', and your team will be gratefull for the 30 AP aura.
