Yeah, only 40% kill contribution and a lot of deaths on average. You seem to have very low impact in your games. I know it's not as easy as it sounds but try to have more impact in games while dying less. Think twice before you do something while staying grouped with your team as a jungler during mid game. Don't really have the time to go in-depth right now because I just wrote a massive comment on my own thread, but feel free to ask if you have some specific questions.
Don't worry though, I've seen worse things ;-) All I want to say is that you cannot blame your team for anything with such death counts and kill contribution percentages.
The deaths come later in the game when I get greedy and just stressed out. My most recent game, I was so on tilt, it wasn't even funny. I probably couldn't even count on my hands how many times people just barely got away. If I ever dove into the backline, I'd get locked down and blown up.

Hm.. I always struggle what to do if the lanes are outplayed and my team mates start to ping constantly at lvl 2. Even if you tell them to just farm safe and don't trade they go all-in and they. Oh, and of course they blame you.
What I normally do is if required I just keep roaming and show my face. With
Jarvan IV I can easy burn their summs or push them back a little so my team mates can farm up which should be a big help already. I think it is also very important to find that jungler that you are confident with. But probably the most important thing is deep warding and keeping your eye on the minimap.
But it is always you who has to make the call to ignore their pings and keep farming or give up some xp/cs and give them hand. Not an easy task. I've had hard time jungling before but now finally I've found the right champ that suits me and so far I've lost only one game with him in ranked which gives me also an extra boost to keep up the good job.

When it comes to ganking you have to read your team mates style before engaging because I have had dozens of times where someone has asked for a gank and I have performed a near perfect one for them only to look up and realise they have used my gank to escape to a different lane leaving me to fight alone under the turret or 2 v 1 which can be a pain in the ***.

If you gank effectively and force them to back enough you should be able to kill first tier tower with the help of the laner.
And once you do that, you can force team fights easily from LvL 6-9 and so the lanes won't be able to lose by themselves,

2- If ennemy doesn't have tp you can do it, otherwise don't you'll probably just steal xp/gold from your laner.
Even then, in some scenarios you can deny more cs from your enemy by freezing the lane. If possible, freezing is the best thing to do as it means your laner won't lose any CS while your enemy will lose just as much as when you pushed it.
However, when freezing the lane is concerned, this is VERY HARD thing to do properly and requires a lot of knowledge on wave control, so don't just do it if you have only ever heard of it.
