Yeah the issue is with Gravatar's internal caching. They cache their avatars and their system is broken - it doesn't refresh the caches when you change it. I don't know why, and I don't know why they never noticed or fixed it, it's a major flaw.
My "hack" solution is to append a timestamp to the URL when you change your Gravatar avatar. This forces their system to make a new cache. So if you change your Gravatar avatar, you have to come back here and re-change it to update the timestamp.
I could put a random timestamp on them every time they are displayed but, this could prevent your browser from caching them, and could piss off Gravatar and get us blocked.
We do have image uploads and are considering a custom avatar system. Just need to be sure it's not gonna smash our database before we do it.
