I'd ban 
Irelia, Gnar, Morgana, Rek'Sai, Azir, Maokai and Thresh
Loads of them are considered just really strong picks at the moment. As I'm naturally prefer playing support I'd opt to ban supports I don't want to deal with such as Morgana because I don't like playing against her kit as you may expect and I'm just afraid of losing a lane to Thresh due to my carry getting hooked in an awkward spot.

It sometimes hinges on what I'm trying to get picked as a priority. Fizz is pretty broken to me, so I try to snatch him on first pick (even 0/3 coming out of lane his roam can stack up kills pretty easily), making me ban Zed and Lissandra first of all, with Syndra coming as a consideration if the opponent bans any of the aforementioned. Last ban will usually be something that's FotM which I consider toxic, like Gnar, Azir and Rek'Sai.
Strong bans right now that affect the whole map will usually be in mid and jungle for me.

I don't always find a threat in these champs since if I'm someone like blitz or thresh I can just destroy kat, but I digress. I usually only see a Thresh/Leona ban if the other team is planning on picking the opposite, since Leona counters every Thresh except me (then again, I do only play with friends who know how to play the game and cooperate well with me). But I was curious, Diamond players, please bestow some wisdom to me.
