I'm not really an adc player, but I believe that lifesteal from the same item won't stack if the lifesteal is marked UNIQUE. As is with 
Bloodthirster, for example (UNIQUE Passive: +20% lifesteal). Buying another Bloodthirster will not increase your lifesteal. Vampiric Scepter's lifesteal is not marked UNIQUE so buying multiple scepters will give you more lifesteal.

well i had a blood thirster, blade of ruined king and a mercuilar scimitar if i remember correctly, after that picked some other item that had lifesteal and it did not add more. i did notice the lifesteal on that item said unique, not sure if that matters. or at least the bloodthirster and one of the lifesteel items i had both said unique, i think i remember reading that could be a issue with stacking

Items like Death's Dance or Hextech Gunblade aren't listed at lifesteal because they are a special form of it.

i am pretty sure the item i bought that did not stack said it had 10% lifesteal, it was not a item like deaths dance or hetech. two if them items said unique 10% lifesteal, i think i remember hearing something about items that had the same unique type of damage did not stack...
Maybe i did buy 2 of the same item, do two of the same itmes no longer stack ??.
looking at the list of items there are not many other items that actually say lifesteal except for sanguine blade (dont think that was it) and ravenous hydra which i hightly doubt i bought since its says i think that items is only for mele.

I have never understood Lifesteal fully really - I have had games where I have ADC and I have 40% Lifesteal and its done nothing and games with 10% where I couldn't heal fast enough

I'll presume that that item you bought was an
Elixir of Wrath. Elixir of wrath isn't quite lifesteal - it's a little bit better, since it heals you based on the damage you deal, rather than just auto attacks. That means that spells that deal AD damage like Volley will heal you for 10% of the damage dealt, instead of just autoattacks healing you.
