By going into your profile > profile settings. In the field for Summoner, you put in your Riot ID so e.g. Name#tag as well as your server and click the green "Update" button at the bottom.
After that there'll be a part in the same Summoner section that says "Click here to begin our automated summoner verification process." -- click the "here" and you'll be taken to the verification section, all you have to do then is to match the random summoner icon you're given with the icon your league account has in-game and then click the "Verify" button.
