I also don't really like the 9/0/21 Mastery setup on Irelia, she is a melee champion and tends to take a lot of damage.. so points in the Defense tree is what I'd recommend. Maybe 0/9/21 (with +6 MR, +6 Armor and 1% of Max Mana for HP Regen/5 would probably be a bit better than 2% Crit Chance, 3% CDR (considering your item build, it's unnecessary) and 15% Magic Pen (maybe when Transcendant Blades dealt Magic Damage.. but certainly not now))

Items:
Shurelya's Reverie - Too much CDR.. if it's for the HP, you can get a much better HP item, and you already have Youmuu's Ghostblade for an active burst of Movement Speed
Ionian Boots of Lucidity - Again, too much CDR
Banshee's Veil - Blocking one spell really doesn't help that much.. maybe only against a very caster heavy team, but I'm sure there's better choices for that than this.
Spirit Visage - Increasing the Healing from spells.. the amount you heal by is minimal, so there's no real point of amplifying that amount. Also the HP is minimal, and also provides even more useless CDR
Thornmail - Randiun's already does this job of fighting against physical damage dealers, Atma's is probably a better choice.


Maybe consider an Aegis of the Legion for your item build, it provides some AD, Armor and Magic Resist for you and your whole team, it is also an extremely efficient item, being cost-effective with its sheer base stats alone.


Skills:
Ionian Fervor - It does NOT provide Tenacity, it provides something similiar to Tenacity, but it doesn't. Mercury's Treads + Ionian Fervor reduces the duration of Crowd Control by 61%
Bladesurge - I personally dislike this, even if you can afford to burn the extra Mana with your Mana Regen.. it'll push the lane back. You might say you're last-hitting with this skill, but the HP you'd be using Bladesurge to last hit would be higher than the HP you'd be using your normal attack to last hit, which would in turn decrease the damage done by the enemy minions, which would keep your minions alive longer.. You see where I'm going with this.
Hiten Style - This skill is one of the reasons why Irelia can stay in lane so long, but more importantly, it's what pretty much gives her all of her damage, one of the reasons why Irelia can build so tanky and still deal so much damage. Even if you don't like it for these reasons, you can still activate it for a Sheen/Trinity proc.
Equilibrium Strike - Also note that you can Stun them if you're both at the same HP % (most notably when you're both at full HP)
Transcendent Blades - Again, note that this skill allows Sheen/Trinity to be proced.. on each cast, as in it can provide a Sheen/Trinity proc on its activation and on each of its casts (remember that Sheen has a cooldown for its activation, so if you time it all out you can deal quite a bit more damage)

Skill Sequence:
As I said above, max Q first doesn't really help you and can put you in a disadvantageous position, now in terms of skill sequence, maxing Q first would either have Hiten Style (W) or Equilibrium Strike (E) at a lower rank. Having a lower level E would lower the duration of the stun/slow as well as not dealing as much damage, and lower W provides less Healing in the laning phase.

Pretty good build i generally build the same items and it works well i like the skill sequences its nice =)
