FMA:B Is essentially following the manga all the way through. As far as I hear it, FMA originally was following the manga, but was being released faster than the manga was, then had to improvise on a story about half way through.

Gantz was horrible imo(But it's one of those "Love it or hate it" Animes), Code Ge*** was awesome!
If you liked Code Ge*** you should try Gurren-Lagann(Awesome), Evangelion(Especially the movies which I like better than the series), Full Metal Panic!, or Mobile Suit Gundam SEED. :)

Yeah, the first episode is the only thing with such gore in it. Wish the anime went a little more in depth to the story though.
Episode 4 is by far the most brutal gore stuff I've ever seen in Animes.
It's 13 Episodes and for this length they did a good job with characters, their developments, backgrounds and dramatic things.

Only watched FMP and GL, but they were pretty decent. Nothing special tho. I think you need to read some quality manga :P
Elfen Lied is one of the rare animes that are actually almost as good as the manga. Pretty good imo.
