An alternative to the Doran blade starter is to start with boots of speed + 3 pots. It gives you a good speed to avoid harass (in particular, it makes it easier to dodge skills shots like Cait's Q) and potentially 600 self-healing power.
If you wanna play safe, only harass with you W : it gives you enough range to pop a few hits safely before running back (which should be, again, easier with early boots).
And, finally... well, having a good support laning with you is priceless. If you lane with Taric, and he just sits behind you popping a heal once a while instead of going front and scaring the enemy with his stuns, you're kinda screwed.

I usually start with boots and 3 pots. I've only ever started with doran once, and that was when the other team's carries were all melee. I generally go boots+pots -> berserker's greaves -> malady -> madred's bloodrazer, and continue on from there. I'll cut out the malady if i'm overfarmed and i can get an early bloodrazer
Imo rushing Bloodrazer is unnecessary : it gives you a great bonus that you already have on your W, while not giving you enough AD to inflict substantial enough damage.
Going on BF would allow you to deal great damage, be sacrier ;
Going on Zeal would allow good mobility, AS and enough crit to deal now and then a scary hit.
I always felt Kog was great with AS stuff late-game only : early game, it's not that effective.

If you're going for mardreds+frozen mallet I suggest starting with
Cloth Armor and 5 health pots, as for last hitting use

Stacking Dorans (not more than 3) is not a bad idea for everyone having too much trouble early. It gives you survivabilty (+300 HP, +9% lifesteal) and still good AD.
Feel free to give it a try, it can be good.
The problem with it is : if it works, it grants you kills, which pay for the 237 gold lost per Doran you will later sell ; if it doesn't work and you still don't manage to get kills, then you just wasted good money you hardly stashed.

Their entire comp was kind of focused twoards that though.

This is mainly an issue when I'm laning against other ranged carries such as Caitlyn. It's hard last hitting while trying to avoid being harassed, or trying to harass back at all. Do any Kog'Maw players have tips on what I should be doing early game? Or maybe how to harass properly? Should I just not harass at all?
