I tried you build and don't really understand it. As a Teemo, I play more AS/Support I also rush for Starks fervor :) Anyway, You get focused way to easy and have no damage in a fight usually..the only thing I could contribute was blind and a little bit of life gain. Your build really seems to hinder your team until you get to the mallet.
Maybe I could recommend changing your masteries into more defense so you aren't as squishy, I understand you want the CDR but being alive seems more important. Your build seems to not be built around doing damage, I'd drop Wit's End (I know it gives MR), but maybe going

Thing is Yes, you may be focused. Yes, you are squishy. But if they really focus you, it's 2v2 fight and you won because they don't focus your carry, or this is teamfight and your team is bad and defending you/your carry. Or everyone from other team is mad at you and they're ignoring you carry = fed Cait is fed. Usually i don't get focused after 20th minute or so(my damage isn't too big compared to carry). Thought i agree i could've more survi, i'll try

9/0/21 isn't bad, it's way more risky than 0/9/21 or yours 0/15/15 thought. In the end of the day, damage dealt by your skills is magic damage.

Ghost/Flash are not good summoners to run, they do nothing for your lane partner.
9/0/21 is bad because all you get is a tiny damage boost from the offense tree. If you want to go 21 utility, go 0/9/21 instead. (I go 0/15/15)

After your Boots, Randuin's, and Shurelya's are completed, you can either build Frozen Mallet+Resist Item, or you can go the starks/will route to really support your strong team.
The goal of support teemo is never to actually do much damage, you are there to soak up damage and blind people.
With Oracles, W, and boots of mobility, you are a ward clearing machine, take advantage of it.
Also, i suggest NOT using gp10 seals, and instead using armor.

Well, i stated in my build that "Support Teemo is funny, kind of viable, thought i don't recommend this for ranked games." ;p. Mostly because he lacks sustain.
