It's because Madred deals more damage the more HP your targets have. Enemies just don't have that much HP before lategame. Lategame is the only place where Madred is truly useful.

It's a BAD item before lategame. Even in lategame you're often better off with AD items than with on hit items because of her W.
It is NOT incredible on her. It's good and ONLY in lategame.
