Prologue
I have absolutely no idea how the item order images turned out so I'm just going to type them here.
Start with:
- Dorans Ring
- 1 Health Potion
- 1 Mana Potion
- 1 Warding Trinket
First Time Back, you should have the following:
- Catalyst
Second Time back, you should have at least a:
- Blasting Wand
Or if not then a
- Boots of Speed
Will suffice
The best option here would be to have completed your Rod of Ages
From there I get: (If I am winning or am quite far ahead)
- Liandrys Torment
- Voidstaff
- Deathcap
- Zhonyas Hourglass
If I am losing then I swap Zhonyas and Voidstaff.
This item build is to allow you the early game health, mana and ability power (Rod of Ages) the Liandrys is to provide constant damage through your E harass and Q, and the magic penetration is always good on Malzahar. Plus when you EWR combo on someone, your W already does a percentage of the targets maximum health, when you add Liandrys and the fact that when someone is slowed, suppressed, or stunned Liandrys will do extra damage your damage from W alone will be immense. Deathcap and Voidstaff are for the ability power spike and magic penetration, which as stated before, are immensely useful. Hourglass is simply for armor and ability power, though if one would like extra burst the use of Deathfires Grasp can be used as well.

Always make sure (during laning phase) that you always have a voidling up when you go in for the fight and ALWAYS ALWAYS ALWAYS make sure you have wards. As a Malzahar you'll push like mad against most champions, so make sure that you always always always have wards.
Your voidlings are your best friend, make sure you have them up. When you have 2 voidlings attacking the same champions when he is under your ulti, the hp melts into nothingness.

My Masteries as seen are 24/0/6, the magic penetration, extra ability power and damage is just insane. The auto attack buffs are simply for harassing and damaging alongside your E in lane.
Runes are for protection and damage, I do swap out my Marks for Penetration ones some times but otherwise those are my standard ones.
