Very efficient at holding lanes
Nice damage per second "aura"
Low damage early game
Starting off with the Regeneration Pendant allows you to remain laned for an extremely long time before heading back to town for additional items. The pendant usually allows me to hit level 6 before going back. Yeah I know some may argue that Doran's Shield is more effective, and yes it probably is BUT losing gold(even very little amounts) early game doesnt exactly give you the edge over your opponents. Choose whichever item you feel more comfortable but I'm going to suggest the pendant.
After purchasing your boots, obtaining Force of Nature will allow you to withstand the constant harassment associated with laning, give you very high magic resistance and most importantly huge health regeneration(which becomes more evident later on in the game).
Ninja Tabi is my preferred type of boots and allows "Nimbleness" to become useful. They can be exchanged for Sorcerer's Shoes for extra magic damage or Mercury Treads to use in conjunction with Burning Agony against a team full of crowd controllers.
Purchase the Giant's Belt first when purchasing your Sunfire Capes. It will allow you to use your Force of Nature's bonus and Dr. Mundo's inate ability to their maximum potential.
Purchase atleast two Sunfire Capes which will give you an aura with a total of 180 damage per second when combined with Burning Agony.
Purchasing Guardian Angel will ensure longer survivability with potential to lead your team into battle with wrecklessness that can win games.
The last item slot is more or less up to user preference. I chose another Sunfire Cape but alternatives consist of:
Aegis of the Legion - increased team effectiveness
Spirit Visage - increased regeneration
Thornmail - damage return and increased survivability versus physical.
Constant Infeced Cleaver harrassment is key to keeping enemies at bay. If health is not an issue and you are unable to hit an enemy champion, targeting low health creeps is an excellent way to push your lane while obtaining some extra gold.
By now you should atleast have one Sunfire Cape now and the damage you receive from enemies shouldn't be that significant. Your damage from Burning Agony and the Sunfire Cape should be high enough to do some pretty good damage and it shouldn't be hard keeping your opponent close with Exaust and Infected Cleaver.
This is where you shine. You are a pushing machine that is very capable of clearing huge amounts of creeps and being the first to head into battle. With a good team there is no reason why you shouldn't win.
TIP: Try using your Sadism when heading into a big team battle, opposed to using it when you're lower health.
