This is simple, all you need to do in team fights to press Alpha Strike (Q), then all enemies will try to kill you so you have to start healing Meditate (W). Let your team know then you are going in by marking one enemie and right away go HAM!!!

What is your best item to get first?
It depends How good you are with poking and in team helping.
First things first if team fight are early, you just have Doran's Shield,but your good at farming, go with Spirit Visage. Get 1400 gold and buy Spectre's Cowl.
If you are bad in all ways get first item Randuin's Omen, get 1000 Gold and buy Warden's Mail. When you get Randuin's Omen and Spirit Visage you can now tank turrets and you will heal really quickly.

WARNING

You MUST max out your Meditate (W) first, then Alpha Strike (Q), your ult Highlander (R) can be used to get faster from point A to point B.

I personally love to take Flash and Heal, because with Heal you can help your team in fights and flash, if you need to escape from enemies.
