750 secoond hit= 2100. almost oneshot on all full adc. Just grab and kill XD
For one, I carried the game. Q, combined with E, and using R in teamfights is just amazing. Not to mention I can safely gather the souls with W, and whenever I manage to catch an enemy, the damage is huge because of the E passive.
But I lost the second one because they fed the top lane (which ganked on bot frequently), and my support was having a horrible connection that he stayed in place and died without doing anything, not to mention Caitlyn is a hard matchup.
This guide is AMAZING. Simply amazing, I loved it. Thresh is a really good ADC.
The only real problem is mobility and range.

One death only, and we were ganked a lot. The enemy team surrendered at 20 minutes.

I've been using Hybrid Pen runes instead of attack damage and Pys dmg quints instead of vamp. More a personal prefernce. Slightly weaker sustain, but much harder hits once you have BT.
I've also been messing around in the trees a bit to compensate for the lack of starting vamp. I've dropped Juggernaught and Vet scars (Probably a bad idea, but it's been working so far.) for Butcher, feast, Spell Weaving and Blade Weaving. As well as swapping Arcane Blade for Frenzy (But I'm still not 100% on that.)
I just Find Blade and Spell weaving to work very well with Thresh's kit and Feast just gives the little extra sustain I find myself missing early one.
Also, Because AD scales so well with Flay, I'd get a second BT over Last Whisper if not considering the alternatives.
Just some feedback, do what you will with it.
Thresh ADC is largely based around 2 things:
Burst and Hybrid damage.
1) You get statikk shiv too early. You should get BT > IE then you check the situation, I usually get botrk or statikk shiv at this point (if enemy team is tanky botrk, if not shiv)
2) Last Whisper should not be gotten. You will usually do 230% of your damage as magic damage on first attack, and all other attacks you will have a 3:1 AD:AP ratio, it's just too much magic damage for last whisper to be viable.
3) Arcane blade is too weak to actually pick
4) In defence take +12% HP, not +2 armor
5) Your killer combo is not really a killer combo, more like an initate (which is only going to work 1v1 as an ADC).
6) Thresh is not a laning ADC but a lategame ADC, and you include nothing about teamfights.
There are many things I could comment on, but all in all I find your guide very lacking.
