You want a heavy dose of AP which in turn gives you a good chunk of health so you can stick it out through some burst. Zhonya's if you have to to prevent fatal damage. The Rylai's can be swapped out for other items (like a Void Staff if you are way ahead, or even a Mejai's if you are slaughtering).
Transfusion (Q) just makes your spell vamp items that much more potent with it's built on additional Vamping
Sanquine Pool (W) puts you into a pool where you cannot be the target of abilities/spells/towers and also siphons more. Try to save this for more defensive reasons, but occasionally you can do crazy tower dives as mentioned in the intro above.
Tides of Blood (E) This ability stacks but initially has a pretty significant health cost to use it. It sends out a wave of mid range missiles that will hit everything in 360 degree radius around Vlad. Stacks of this can increase his spell vamp by up to 8%.
Hemoplague (R) This will allow you to put extra hurt on as it increases damage done to any enemy effected and does an initial burst of damage followed by another burst at the end of it's duration.
Ghost (Summoner 1) I recommend this as it works well with the W and allows you to slow more targets or get away that much faster.
Smite (Summoner 2) This is tossed into this build because it is up much more often than Heal Summoner spell and does plenty enough to get the job done when you are in need of a quick chunk of health back.

Only try this if you are used to it with a few games tested and tried. Vman has played several rounds of Smite Vlad and knows more of the intricate ins and outs of what is needed to be successful with this build. That being said, if you do not pull this off you could end up being reported for trolling etc, but I've seen this build work magic and was really impressive on stream against other Diamond level players.
