Introduction
(KEEP IN MIND: I am not the best player ever. I don't often play ranked, but This build should still be VERYYYY effective in ranked.)
The Greater Seal of Potency provide .59 flat AP. This is a nice start to a game with a flat 20 AP.
The Greater Mark of Insight is a MUST .95 Magic Pen per = 8.5 flat magic pen. This is also great for start because your poisons at level 2 will hit for more. (this will pair great with void staff and sorcerer's boots.)
Finally, the Greater Seal of Force provide a small AP per level with .17 per level, coming to about an additional 23 AP at level 18. This will help scale AP a little bit more while leveling and will give you overall more damage when paired with low level/money acquired gear.
(NOTE: the Greater Seal of Force can be exchanged for Greater Seal of Potency for more flat AP instead of scaling, making you better early game, rather than late game.)

Start with Boots and Potions. Then go for the base items for a Nashor's Tooth. Then after you finish the 2 base items, finish boots for Sorcerer's Shoes. Then finish off Nashor's. Moving onto a Malady for speed and an on-hit. Then move on to Void Staff for Magic Pen to go with runes and boots. Then, grab a Rabadon's Deathcap for pure AP scaling and maximum damage. Then, finish with a Rylai's Crystal Scepter for Health, AP, and a slow. These items will provide amazing damage with some nice spell pen, and decent HP from Rylai's making you a LITTLE more sustainable..
Teemo's very amazing ultimate! With this build, your mushrooms are insane damage and a nice slow. End-game your mushrooms do about 1.2k damage over 4 seconds (without calculations for thier Magic Resist and your Magic Pen.) There are areas that you need them in, in order to protect from ganks, baron kills, and dragon kills (and any other buffs: red, blue, etc...)

I use Ignite and Flash. Ignite for early game when HP is low and you wanna ensure that kill, or just for extra damage with those poisons! Flash is for ganks or escapes (AS ALWAYS!!!!) ignite can be exchanged for a more comfortable option, but i recommend keeping flash. Ghost isn't important because your W is already a ghost and Teemo is the League's fastest champion.

NOTE: These 3 games were played in different settings.
Game 1 was a custom 5v5 on summoner's rift made up of all random players in a game that I created, they were all level 30 with runes and team structure.
Game 2 was a normal random game 5v5 on summoner's rift of only level 30s.
Game 3 was a 1v1 on Proving Grounds(Which doesn't show much because its easy to build against the enemy.) against a very skilled Ashe (carry vs carry) We both build pure carry with no defense (I followed my build.)
It proves to be quite effective and I am sure it will do the same in ranked.
*If you are carrying then you want to get those last hits. Farm some gold, get fed! You can (ONLY WHEN YOU HAVE YOUR MUSHROOMS IN NECESSARY SPOTS, ie: Baron/Dragon and brushes) use your mushrooms when a lane gets SUPER heavily flooded with minions. This is only useful late game when minions can potentially push your tower.
Since you are ranged you want to HARASS the enemy hard. Do NOT let them kill minions or get last hits.
*If you are support/carrying then focus on keeping mushrooms in brush for any potential flanks and ganks, and first hitting minions to weaken them for your partner.
