-Ghost Flash. Big nono. Getting Ghost on flash is like giving Sonic the Hedgehog steroids. The mofo runs fast enough, if you enhance it, oh god. It's literally overkill. Exhaust/Flash is prime. When you anticarry/burst someone down, Exhaust will win you the fight. Whether it's you turning around after 'running' to kill them, or to make sure they can't escape you, it's that. Exhaust also turns a champion into virtually nothing. I know you list Exhaust/Flash as an option, but it literally should be your #1 option. Cleanse is mix and match depending on enemy team (applicable in ranked primarily since you see a sneakpeek at enemy team.).
-Doran's Blade stacking. Ehhh, no. As an AD, you want to rush that Infinity Edge or Zeal. IE for the real damage, Zeal for the easy gank/mobility and extra attack speed for more easy harassing with AA and Silver Bolts. Doran's Blade stacking just isn't worth it as Vayne. If you are really scared about lane sustain at level one, just get shield. Shield > Blade anyday.
-Skilling. You want to max Tumble first because of the reduced cooldown and more easy harassment. Although maxing Silver Bolts sounds good, do you think that you'll get that many 3 shot combos on people are Level 9? If yes, your enemies must be pretty bad. Even if you're ganking, you'll get 1 smooth combo off, 2 with a Condemn. With maxing Tumble, your escape-ility increases, more extra damage, and it's reliable damage. Silver Bolts just relies too much on that third hit. When you harass, people will let the timer run when they're at 2 stacks.
Besides that, there is nothing else that stands out too much at 2 AM >_>

Xaioli, not sure I agree about the bit on Doran-stacking. The High Elo guides on solo mid recommend it on TF and I find in terms of squishiness that Vayne/TF are on the same plane. I mean, as usual, it really depends on how things are going. If you're just raping right off the bat I would agree that stacking Doran's isn't so valuable, but if you're finding fights ending with both parties running away really low HP you getting a couple Doran's would switch that to you walking away with more HP and, quite likely, them dead.
I'm pretty sure I agree with you on 'q' vs 'w'. I did some trials and found that I was doing far better with 'q' maxed over 'w'.

Thanks Xaio, I kind of agree with you now on the Exhaust/Flash combo, mainly because I didn't realize the mastery point for it gives you 10 extra armor pen... that's pretty ridiculous on Vayne. I don't know though, I'll have to test out how it works myself -- seems like it wouldn't be as effective of a survival tool against ganks.
I think I'll change the skill order too, I never actually tested that skill order myself but it seemed to make a lot of sense. Looking back, that skill isn't very devastating until you have a lot of AS.
As for getting an extra Doran's Blade early, it's cheap for the stats, it makes last-hitting easier, gives you a bit of a buffer against aggressive players, and gives a bit more lane sustain. I've seen a lot of extremely good players get two, maybe occasionally three on Vayne and I think it's just to make up for how easy she falls over. I think I'll keep the Doran's stacking, but I'll probably change Exhaust/Flash after I try it a few times.

Yea. They give a lot of stats for the price (HP, lifesteal, AD), the reason they are so cheap is because you can't build them into anything later. But they give a nice early game boost :)
