Always unlocked,
You have the advantage of looking ahead and get somewhat an extended sight range.
The only reason why someone would keep it locked, is if they can't control the camera angle with the mouse. Like say my little sister, just an example.
PS: I guess you got a headache cause you're not used to it yet, and probably got a very sensitive mouse setting.

Yeah. It's probably because I'm so used to my character being centered, so it was weird trying to follow Annie around and not being centered. I'm probably going to play some practice games to get used to it so I can be better and stuff! (even though I don't even do ranked games XD)
It is not possible to play with locked camera... i just use space when i need to. Get a better mouse that is fast enaugh and acurate if you have problems. But moving your screen should be neutral thing to do.
Try aiming Ezreal, Ashe or Lux ulti with camera locked... lulz

I tried unlocked for the first time about 2 weeks ago. I'm just now getting used to not spamming random buttons and flippin out when i get attacked or something big happens. At first you can sometimes die without even knowing it. O.o
Still going back and forth a lot though.
