The thing is, your flat penetration comes into play first, and then your percent penetration. So you already have around 55 MP from Haunting Guise, boots, and runes. Most champions don't have anything about 50 magic resist unless they buy items, so you'll generally reduce MR to zero with just that, meaning that your Void Staff is useless. Against a character that has 100 MR, your Void Staff is still only taking out about 20 MR.
Haunting Guise is really only useful early game, to be honest. I would drop both the Haunting Guise and the Void Staff and put in an Abyssal Scepter and perhaps a Zhonya's Hourglass.

Ah, but Abyssal Scepter is flat reduction, not percent reduction, so it's still 100% useful against all enemy targets. In addition, by removing Haunting Guise, you have around 55 MP, which is about how much the average champion has. Even though it costs more, it gives you magic resist, which makes you beefier, and has an aura that helps your team out.

The thing is you're wasting a lot of your void staff's use because you have so much flat mpen. Flat mpen is applied before the %mpen is applied, so the more flat mpen you have the less your %mpen will do. With this build on anybody but a tank your %mpen will be useless, but with Abyssal Scepter since it's Flat mpen it will remain useful.

I dont like the hourglass with swain, its just useless against mages.
I think more AP from Deathcape adjust the less Armor with Swains Ulti.
It's not for the Defense or AP but rather, the active. On a pretty defensive character like swain (good HP pool along with spell vamp to keep his *** alive and pretty good early game burst), the active from Hourglass will save your life more than you can count.

I think the hourglass active would definitely be mostly viable if your ulti still works while in the state of hourglass' active. Since I haven't ever played Swain I wouldn't know. If it somehow cancels the effectiveness of Swain's ulti however, I'd bow towards calling it rather useless.
I think the idea, of going into a team fight, getting focused, popping ulti when low, and using the hourglass active seems to be a nice combo, that'll leave the enemy waste a lot of time and such.

I'd go with
Will of the Ancients of you want spell vamp. Spirit Visage has been nerfed a lot.
