Just wrote up a new Hybrid Twisted Fate build -check it out!
To elaborate on Nighthawk's post, just in case it was unclear:
Say your champion naturally has 100 AD at level 18, and has an additional 300 AD from items.
Trinity Force adds on an extra 100*1.5=150 damage to your current 400 attack damage, making your next attack deal 550 damage.
But yes, your question should have been answered - it's an ADDITIONAL 150% instead of just increasing it TO 150%.

You are looking at it a bit wrong as Jebus already suggested. Dont look at your AD, since your current AD doesnt affect TF at all. It is the BASE AD that is considered.
The procs from TF and LB are Physical damage and gets decreased by armor.
Calculations on damage are in 2 steps. 1st you get the normal attack damage you would do than it deals the damage from TF so:
If you have BASE 100AD and 300AD from items with total of 400AD in stats you would do on a target with 0 armor 400 damage than another damage will pop up and this will be that 150 from TF. Both will get Armor penality counted separatly rounded down.
