The passive should read:
"Because of your passive enemies will always focus you as the number one target to stop you from boosting your defensive values. This will force you into playing your champ more tanky than you want to."

His in-combat/out-of-combat status should work the same. Like someone else said,

This means that proximity to combat will not trigger the passive. You need to be attacking or taking damage in order remain in combat.

I'm puzzled, when is a char considered in combat again? He just has to attack or he has to have at least minions aiming at him?
I would assume that the "in combat" effect works in the opposite manner that Garen's passive works. That would mean that it would be in affect as long as he has dealt or taken damage in the past 'x' seconds (Garen's passive says 7 seconds but who's to say his would be the same).
