I think part of the problem with Sejuani is that she lacks the hard CC Leona, Shen, Ammumu, or Rammus have. She works more like a Mundo tank that mostly just slows people instead of acutally pulling them away. Now this does not mean she can tank, and be effictive there are just better choices imo.
I mean she has her ultimate, but past that the most she can do is slow. So people with gapclosers can easily catch thier prey.
Spinning Slash and
Leap Strike for example.

The idea that makes a tank, a tank...Is that they deal sensible damage if left alone, but are also extremely durable.
Sejuani has no built in mechanic to make herself MORE durable. No shield like Leona, no health steroid like Chogath, no bonus MR/Armor/CCR etc....Building Sejuani pure tank makes about as much sense as building AD Annie. Can it work? Yes, but is it exploiting her potential? Nope, not even remotely so.
That isn't to say Sejuani makes the greatest OT ever, but you need to build a champion to exploit their strengths, not to turn them into something they aren't...
That's just my opinion. Take it as you will.

Frozen Heart (great cd , mana and armor)

Frozen Mallet (depends if I need the extra slow when random people do not react quickly

(of course this is not set in stone, depends on what I have to counter, but this gives you a general idea)

Stay alive long, disrupt their team, slow them all the time, stop chasers, and stop them following your team. Usually if you stay alive long enough you have an amazing damage output anyway...
Due to your durability and your Q-W-E-W combo or your R-W-E-Q-W-E combo you deal tons of sustained damage in teamfights by just staying alive and dishing out crazy aoe...
I have not found a really solid guide for her and as much as I would like to write one, I will wait until I have player her at least 50 times or more (right now only about 20+ games) and will wait for all those overeager people to abandon guide writing about her as it is simply a stupid rating battle atm...

Well honestly, I think she's a ganker. Her early game and mid game ganks are good, but late game I find that she lacks a bit in every field (damage, survivability, cc).

Of course she is a great ganker with two initiating tools and slows from hell. But even late game she can easily change the outcome of teamfights by using her ultimate and helping to stop anybody from escaping or repositioning while dealing dot all the time...

Sejuani can do early ganks because she doesn't need red buff. Her Q does it for her.
She initiates team fights but her damage is almost ignorable in fights unless you build her AD. Not the best route.
Her early game is terrible bu she can pick off runners in a 1v1 unless they actually fight.
Honestly, she's a little weak without some hard CC but she's an amazing ganker past 6. Probably a better ganker than WW post-6.
