While you are editing your guide, there is window on the right titled Guide Setup containing quick links to add recommended / custom chapters. Below this is an advanced section with a check box which - when enabled - turns on "Require comment to vote". Please note, this feature only affects the first 20 votes. After your 20th vote it will no longer apply and voting will be anonymous once again.

It looks like one, did a quick check and it looks like you chose everything needed, enough characters... have you set what kind of guide you want it to be? I believe it is close to the save/publish/view buttons.
