Hmm, You can start at blue no problem if you start with his E. I've had some success with going AP DPS since we already had a tank and I was bored. His ganks aren't that strong though as you need to time your q well, but if you have red buff you can stay on em like a hound dog if you land it.
-Flash or Ghost?
I feel like ghost is stronger on a jungler. If you have red buff it might be the difference between a kill and nothing.
-What the hell do I build an Amplifying Tome into on Cho'Gath? Do I just ignore it until late-game, when I turn it into a Rylai's?
Well, I personally build him AP DPS... when I jungle with him, so I build it into nashor's, CDR on him is great and the AS will allow you to stay on someone with red buff. You can also build the rylais as that procs on all your abilities =P
-Mana regen, flat health, or armor Seals?
If you're going to have blue buff constantly I'd go with armor seals as they make your health more effective. Otherwise mana regen is always good, a Cho with no mana sucks...

You can start at his blue, but he has to recall after finishing red/double golems, meaning that he has less time on his buffs. Stonewall suggests the route I posted because it's safer and gets you to level six much more reliably, while being able to gank at any time.

E-Q-Q-E-W-R
Then maxing Q first, followed by alternating W and E, prioritizing W so it's always one level higher or equal to my E. Obviously get your Feast whenever you can.
One thing I love about Cho'Gath is that it's almost impossible to counter-jungle or smite steal from him mid-game. He can nuke for around 1600 damage in a split second, even on Baron.

^This. Sometimes I also do E-Q-E-Q-W-R for sustained mana for ganks.

-Excellent AP scaling with his Q
-Great farming ability with his E in the jungle
-AoE silence and stun
-Ability to deal upwards of 1600 damage in a nuke against all minions
The last aspect is the most appealing, as you can run into their jungle and steal a buff almost as fast as Nunu can, if not faster. He's a great ganker too. I've also discovered that you can basically start with any item on him, as long as you follow this jungling route:
Wolves->Wraiths (smite)->Double Golem->Blue pill back (buy 4-5 potions)
Blue buff (smite)->Wolves->Wraiths->Double Golem->Lizard (smite)-Gank or blue pill
That brings you to level 6, and it's very safe.
I've been starting with an Amplifying Tome, as with MPen marks, you meet a lot of sweet spots such as just barely being able to take out wolves and wraiths in fewer attacks than you normally would need.
