With this build, you're meant to poke down the enemy support and ADC and outheal their return poke when able. starting at 15% cdr puts your q at a 4 seconds cooldown level one, and with the Ancient Coin giving you decent mana regen, you can spam it decently often. Try to save your heal for when you land a q so that your ADC also gets the rejuvination from your q passive, as it gives health over time and a little extra movement speed.

You become a decent powerhouse. Whether it's chunking the squishies with your q's, or peeling with a Rylai's e, You should be able to disrupt a lot during a teamfight and still get off the heals you need. This is my preferred build because between the extra healing with Spirit Visage, the extra healing you do with the Ardent Censor Procs, and the damage/healing you output with having almost 200 AP, it gives you a chance to bring a new meaning of an aggressive support.
