here is how i run a Dr. build don't think it is anything fancy, get some hp regen then throw cleaver into people, making shure to hit minions if you miss so you can gain the hp back from hitting a target. get the spell vamp up to gain HP from your W spell, once you get 170ish hp regain you can have W up all the time and cast it for free never needing to take it down unless you need to stay alive better.
