Introduction (and why you should read this guide)
Hi!. I've been playing Milio and I didn't like any other guide or "meta build".

The first issue: most are using Dawncore to scale shields and heals, but they scale way better with ability power.


DAWNCORE
VS

HORIZON FOCUS
Dawncore improves healing and shielding trough heal and shield power and unique passive. Unfortunately, this mechanic has been nerfed from quite some time Horizon Focus improves healing and shielding trough ability power scaling, which works way better than the alternative. AP also empowers Ultra Mega Fire Kick (Q). It also provides a lot of Ability Haste





Second issue: many guides use several healing items ( Echoes of Helia + Redemption). Keep in mind that healig scales poorly, and Redemption is flat healing that got nerfed. Good on itself with some supports as first item, but we can heal more than enough between Heal, Echoes of Helia and Breath of Life (R).

Third issue: redundance. Why use Mikael's Blessing or Redemption when we have Breath of Life (R), why Locket of the Iron Solari when we have Warm Hugs (E), why Staff of Flowing Water when we have Fired Up! (passive)

Fourth issue: Ardent Censer is situational, only good with ADCs that scale fast with Attack Speed. It's not a terrible choice but other items with more AP and AH can take it's place. Take it and it's alternative item path if your ADC scales with AS. Check Alternative paths for a list of potential champions.

Basically, I refined the items, runes and abilities for the last patch based on my experience and focusing on shields

Abilities
PASSIVE
ABILITY
FIRED UP! (Passive): Every time we interact with an ally, gives a buff for 4 seconds that empowers both Milio and our ally with extra AP

Q
ULTRA MEGA FIRE KICK (Q):Our Q will be mostly for disengage and can give our passive to any ally it goes through. Get this at level 2 if you are woried the jungler will rotate bot before your level 3. You can also consider improving (Q) to level 3 to one shot ranged minions once you have Horizon Focus to farm alone in a line.

W
COZY CAMPFIRE (W): This, to me, is the biggest trap of Milio. Cozy Campfire has a very bad scaling (%15 AP). It's very useful to activate Fired Up! and it also gives attack range, but it's overall the weakest ability of our set. Get this at level 2 if you want to heal and empower your long ranged ADC and have no fear of ganks, but leave it at level 1 until endgame.
However, if you are playing with long range ADCs then consider the alternative path of W as your second basic ability to max out.Check Alternative paths for more champions

E
WARM HUGS (E):The core of our build is this ability. We will choose items that empowers Warm Hugs and Breath of Life through AP and AH.

R
BREATH OF LIFE (R): This will be our main healing tool in the late game, since it's the best at scaling among our options. The clensing is also very good.
Runes

RESOLVE
  • KEYSTONE
  • Guardian: Nice for Milio in the early game since it can shield champions who stays besides him. We'll take it as our primary rune if we have an ADC that shoots from afar and expect ether poke or ganks.
  • TIER 2
  • Bone Plating: Consider by many the best option to keep if Resolve is your secondary rune. It gives a bit of sustain, specially if the enemy on bot lane has long cooldowns or the enemy jungler is an assassin.
  • TIER 3
  • Revitalize: The best rune for enchanters. Must keep it if you use Resolve as the secondary rune.



SORCERY
  • KEYSTONE
  • Summon Aery: Great for Milio since it will add a shield that slightly scales with AP. We'll take it as our primary rune if we have an ADC with low range and loves to dive-in, like Kai'Sa.



DOMINATION
  • TIER 2
  • Grisly Mementos: Helps with vision, not to be underestimated. Sixth Sense also helps with vision, altough I don't think is as useful.


Ability Haste is a go-to since it's important for Warm Hugs and Fired Up! in general. Flat Health comes at a time you are the most vulnerable, while Health Scaling doesn't scale nicely.



INSPIRATION: As your secondary rune you can also use Inspiration with Magical Footwear and Cosmic Insight. I don't like Biscuit Delivery since I feel gives too little. The benefits are less than ether of the other options so I don't use it.
Items

Dream Maker

Echoes of Helia

Horizon Focus
This will provide all the healing we need, on top of a big amount of ability power and Ability Haste for cheap. Once we have this our shields are going to be powerfull and we can use our passive quite often. Use Horizon Focus before they nerf it! We can also apply it's passive with Q, granting us vision to an enemy and area around them.

Ardent Censer
We'll rush this item if our ADC can make a huge difference with the extra Attack Speed. Think of champions like Caitlyn with her passive. Check Alternative paths for more champions

Ionian Boots of Lucidity

Boots of Swiftness
Between Ultra Mega Fire Kick and Warm Hugs I don't usually suffer much poke or gank. Ionian Boots of Lucidity should be more than enough but you can change them for Boots of Swiftness for the extra speed and some resistance in teamfights. Same thing for Mercury's Treads if you are more comfortable with a bit of resistance and tenacity

Moonstone Renewer

Malignance
Moonstone Renewer is always nice to have on teamfights, but since W and R are AoE and we have 2 charges of E, is not the power spike we can have with other champions. Malignance is good for the ultimate ability haste, since is our most powerfull heal and also cleanses in AoE. We don't have any item like this but for enchanters, so we loose a bit of functionality, but is nothing big.

What about the "Latest of games" build?
Once the game has drag for too long (after 30, 40 minutes) we want to discard Echoes of Helia and Dream Maker since they provide a flat amount of healing and damage, wich at this point should do no difference. Moonstone is similar, now we are close to level 18 with a lot of Ability Haste, so we don't need help to shield, heal and activate our passive. The 35% shield is not that great at this point.

Imperial Mandate

Rabadon's Deathcap
We want to replace those items with something more usefull, if possible with percentages. That's where Imperial Mandate and Rabadon's Deathcap came into play, giving a % of damage and a % of AP. Zhonya's Hourglass, Banshee's Veil and Morellonomicon are situational, whether we are being focused by the enemy team for our incredible value or just trying to add Wound to them.
Alternative paths
Here is my atempt at clasifying ADCs among those who get a lot of benefit from more attack range or Attack Speed, to evaluate if you should change ether items and/or abilities.

Consider this a WIP and also subjective, and pair this info with the Threats and Synergies section, as well as how the early games plays for you.
