Why Top? In my opinion it's just as good, if not even better, to start roaming with well placed tunnles.
Spirit Visage? The spirit visage increases your fury to health active when burrowed.

Farming with Rek'Sai is easy as hell. Just use your q for the last hits and you will miss almost no creeps and even heal afterwards, if needed. No mana ftw.

If you are sure you can kill the enemy: Unburrow on them, q and once the rage is up e.
If they try to run, just place a tunnle. There's no escaping Rek'Sai.
Once you start roaming, place tunnles near every lane, so you can ult to them and get easy kills or give your teammates easy kills.
Defending and securing objects is also pretty easy with Rek'Sai, the tunnles make roaming way too good.
