You really want to try and zone the enemy in solo lane out. When there is a two person top on the enemy team then you try to stay back till like lvl 4 where you can still get poison our on minions and fling them into turret if they get too close. You want to keep going back, if lane is ok, to get potions. You won't need to get potions after you get catalyst or Rod of Ages.

You want to be the person that engages the fight in all team fights. Once you have some tanking items you can easily go in with ult and have no problem tanking team fights. Just make sure that no enemy runs away and when they do just fling them back into the fight or slow them with your adhesive. Also always leave poison on in fights.

This build has always worked for me and I've never had a game with more than 4 deaths. It's not based on how many kills you get but how much you fed your teammates and are able to engage with no worry of dying.
