Alright about all of the potions you don't need them if you get a good life steal item in the beginning and stack bloodthirster instead of getting a second one get phantom dance for critical.

I thought your guide was well thought out, except for a few things:

Instead of Ninja Tabi, I get the Sorceror's Shoes as this gives you good magic penetration, which is what Teemo's spells are all about. Madred's is a good item, but don't get it if there are no tanks on the opposing team. If there are lots of mages on the other team, get a magic res item.

Recommended summoner spells are okay, but if don't list revive as a good spell. Revive is a huge waste of time as the cooldown time is ridiculous. If you want to play an offensive Teemo, Ignite is a good one to use, and Ghost if you want a FAST Teemo. Exhaust also works wonders because of your fast attack speed, which will shred them to pieces.

A technical issue: Malphite doesn't throw a barrel, Kragas does. Malphite's skill is called Seismic Shard. And any hero with a stun is bad for Teemo, as they can open up with it and then do some major damage (since Teemo is so squishy)
