Almost impossible matchup when against a good Zyra. Aside from being able to poke you and root you from being able to walk up and run her down, she also has plants to block your hook. She also has nasty counterengage with her R.
Yuumi
She's only here as 5 assuming she is paired with Ezreal or Lucian or another ADC with a gapcloser, since you would actually have to predict hooks or else not be able to do anything but get poked and outscaled. Otherwise, Yuumi really isn't much of a threat as you can usually win all-ins just by walking up and running her ad down.
Morgana
The classic counter against hook champs is still no fun to deal with. She's actually a great ban as she is so commonly picked into Thresh. Playing against her is mainly baiting her black shield and dodging her binding. Like Zyra, she also has good counterengage if you can't kill her fast enough.
Brand
Brand has high damage, good poke, and can easily kill you if he gets his combo off(usually E-Q-W, sometimes with R and ignite). Playing against him is mainly baiting/dodging his Q after he uses a spell, and all-inning after.
Vel'Koz
He's on here for basically the same reasons as brand, except arguably he's a bit harder to deal with as his Q slows.
Leona
You can flay her out of her E and poke her out. However, Leona is still the stronger all-in champion if you can't stop her E, as she basically has 2 stuns and 2 Aftershocks.
Xerath
He's a mage, but probably the easiest mage matchup you'll deal with compared to the other mages, since a full spell rotation from him does less compared to the rest of the mages. But he is really cancer to play against and he makes me very, VERY ANGRY.
Pyke
This is a skill matchup, you can flay his Dash and his Hook, but if you let him get his abilities off he will kill you or your adc.
Nautilus
Like Leona, if he gets his full engage off he's usually going to win the all-in. You should be looking to harass him or his ad early on, and then be looking for all-ins after he has his hook on cooldown.
Blitzcrank
Don't get hooked and actually land hooks = Free matchup.
Seriously, if he lands his hook he probably will win the all in, so play around his Q cooldown. Also try to not walk around by his turret without minions, because he can hook you under turret for an easy kill.
Nami
She can be oppressive, and is often paired with annoying mage adcs such as Syndra or Cassiopeia, but if you can find an engage without getting too poked out you'll most likely win. Playing around her Q is also recommended.
Alistar
This is also a skill matchup, you can flay him out of his headbutt to cancel his engage and kill him right after, but if he baits out your flay you will lose the all-in. In that case, try to at least land your hook so you proc Aftershock and don't instantly die.
Bard
He has high damage and good harass early on, and his Q can stun you or your allies to your lantern.
However, he is extremely susceptible to engages and pops just as fast as any egirl if you find a good engage. Just don't forget he can often portal his Jungler along with himself to lane to flank you.
Lulu
She has really good harass early on and can stuff your engages by polymorphing your ADC or ulting whoever you are focusing. However, she's also really squishy and dies fast if you can catch her out without those aforementioned abilties.
Janna
Janna, even without Omnistone and with Comet and W-max, is still not exactly the best laner. You can try to flay her when she attempts to W, or at least try to trade back with an flay-empowered auto. Janna is extremely easy to kill as long as you can hook her or her ad, as her shield really doesn't provide much early on.
Soraka
After nerfing her to prevent her from being a Laner, support Soraka hasn't really been able to be aggressive as before. Although she's pretty vulnerable to all-ins, you still probably shouldn't focus her AD without ignite.
Swain
Swain is extremely hard to kill after he is lvl 6 due to his ult and stopwatch combo. However, he isn't hard to kill before then, just watch out for being snared by his grab, as he actually has pretty high burst damage. At level 1 he also has he E where he grabs yo balls and that **** hurts, like wtf man.
Rakan
He's only here because playing against Xayah Rakan with a random adc can be really hard. Rakan by himself really isn't much of a problem, although I would suggest baiting out his E before fighting, and keep in mind you can cc him out of his dash.
Sona
Probably the easiest lane for you, you probably won't lose any all in unless they have 69 more minions than you.
Lux
She's somewhere between a 3 and 4 as she has decent poke and can root you, and has all the qualities of an annoying mage matchup, but she really isn't as hard to deal with as Velkoz/Brand/Zyra.
Heimerdinger
Good luck doing anything when he can block hooks with turrets, and if you take any hooks you'll probably just die inside his turrets. No worries because only psychopaths play Heim.
Shaco
Basically not hookable unless you predict, has insane amounts of counterengage, decent poke, has everything there is to stuff hook champions. You can't play lane but Shaco doesn't stop you from roaming.
Braum
He thicc with his big, BIG shield. Braum can be hard to kill and might even kill you if you engage on him while he and his AD are still high HP. However, you can harass him down really hard before you all-in but that's difficult. You can also flay him out of his dash, by the way.
Tahm Kench
Really similar to Braum Matchup, except his lvl 6 is much weaker but you have to either play around his W cooldown or get him low enough to kill him first.
Senna
Her poke combined with glacial augment can be really annoying, but she is extremely vulnerable to all ins.
Seraphine
If you're not careful in lane, you can poke you down really hard. And her ult makes your whole team ***** which is very VERY obnoxious >:(
Sett
He can kill you if you misposition and don't harass him enough. He is also a Jojo's character so you if you play in JP Server you auto-lose.
Pantheon
If you can't flay his jump, you lose. If you can flay it, you win. Squishy alistar with more dmg.
Rell
Don't underestimate her, but you can flay her jump which makes Rell look very stupid hehe. But level 6 be very careful since her ult should win all-ins for her.
Kalista
Kalista is only here because I have been held at gunpoint by a Kalista one trick (HELP ME).
Kalista has extremely high kill potential in lane, and is naturally very aggressive, both of which match well with Thresh. She also has a slow, along with an ult that would make you almost ungankable after lvl 6.
Lucian
High kill potential + aggressive adc = :), but most Lucians suck dik
Draven
High kill potential + aggressive adc = :). Draven does require more peel than the others, as he has lower mobility and his positioning is telegraphed. Draven mains typically have large quantities of autism as they follow their lord and savior, Tyler1.
Varus
Varus is somewhat of a lane bully, and he has a slow and root.
Caitlyn
Lane bully, and a good Caitlyn can trap off of your hooks/flays, meaning high kill potential. You can also flay/hook targets into Caitlyn traps.
Ashe
Ashe has consistent slows with her W and Autos, and usually Ashe runs Approach Velocity, meaning she can keep running and slowing enemies down, making enemies easy to CC. Her ult is also a stun, making your cc abilities impossible to miss.
Jhin
Jhin can W root off of your CC ablities, and sometimes can slow/root off of his traps, but not much synergy otherwise. 4
Miss Fortune
She's a lane bully with decent burst and a weak slow, can be a good lane occasionally.
Tristana
Tristana has high kill potential at lvls 2 and 6, but usually can't take trades like Lucian Draven or Kalista.
Kai'Sa
Kai'sa can usually kill if she fully stacks her passive, but she isn't exactly a super high damage or aggressive adc during the lane phase.
Aphelios
Aphelios has quite a lot to work with Thresh, he's an insanely cancerous disgusting subhuman champion even after his 69 nerfs.
Sivir
Sivir has low range, isn't exactly lane bully, and is probably always shoving waves under the turret, which makes it pretty hard to find all ins. Assuming you land a hook, Sivir's low damage output early makes it hard for her to kill.
Kog'Maw
Kog'Maw is just a straight up horrible champion for soloq without an egirl support. Without extreme shields heals and peel, he really isn't a champion. Go play lulu or yuumi with him because Kog'Maw only works when you simp for Reddit Egirl.
Ezreal
You will not find a single decent Ezreal that does anything in soloq until you get to actual high elo. Like at least D1+ I'd say.
Half-joking aside, you realistically want to look for all-ins after Ezreal pokes enemies down, but this usually doesn't happen often. After Ezreal's W-E burst, his DPS compared to any other ADC is noticeably lower, even if he is autoing and landing Q's.
Jinx
While she does have a slow and root, Jinx is kind of weak early on, making it hard to find winning all-ins in a lot of matchups. Also she flat :/
Xayah
She has good roots that synergize with Thresh's kit, but she has a weak early game and has been out of meta. These few factors including the fact that she has a nose piercing renders her only a strong compatibility with Thresh x3

Hi, I'm "5haark", a diamond NA support main(ignore my mobafire name pls). I currently have over 1 million mastery points on Thresh(Look up "Not Unwell", "5haark", and "Not Unweii" and add those mastery points up), and I have one-tricked him since I started playing League until fairly recently, where I have started to branch out and play other champions. Even now, I would tell any support main to at least play some Thresh, as he is not only one of the most popular one-trick champions(and therefore one of the most common matchups you'll run into), but also just in general a great pick up as he's probably the most versatile champion in the game, meaning that you'll be able to run him into most matchups, and always be relevant no matter how the game is going.
Ablities
You should be starting E(flay) lvl 1, unless you are invading, and then level up hook for level 2, and W(Lantern) for level 3. After that you want to Max R > Q > W > E. Optionally, you can put one or two points into flay for extra harass and increased slow, but again, this is up to personal preference.
Here's a basic rundown of all your abilities: Q, or Death Sentence, is Thresh's hook. This has a 0.5 second delay on it, and is slower than the other hook supports' hooks. However, this hook is harder to buffer by abilities such as Tristana jump(W) as it has 2 tugs/pulls to it. You can also re-activate this to pull yourself to the hooked enemy.
W, or Dark Passage, is Thresh's lantern. You can throw the lantern for an ally to click, and it will bring them to you. The Lantern also provides a small shield, which can proc Guardian, and sometimes it can make the difference in close 1v1 fights.
E: This is Thresh's flay, where he can either push or pull on enemies. This ability can also be used to cancel dashes(not blinks) if timed well. You can sometimes opt to engage on enemies by flaying them towards yourself first, and then following up with your hook.
R, the Box, is Thresh's Ultimate. This is by far the simplest ability in his kit, and is mainly used either to zone off areas or used after you land a hook to guarantee a slow. When someone walks into a wall of the box, they are slowed by %99 and take some magic damage ***suming they are the first to hit a wall.
Item Build
Thresh actually doesn't really have a set order to build his items in, as they are basically all situational. As of right now, however, I would recommend rushing Mobis or Swifties(depending on how much gold you have during your back)before going into the other items
Here is a Basic Rundown of the rest of the items: Redemption is never a bad buy, as the active ability and stats are all fairly useful for Thresh
Zeke's is good with Attack Speed based AD Carries, especially if they are ahead.
Another Item that is almost always good is Locket, since the extra shield can often times make or break a teamfight. MR and Armor is always welcome on a somewhat tanky champion like Thresh.
Knight's Vow isn't exactly the most cost-efficient item for Thresh as the the passive is reduced for ranged champions. However, this is still a solid pick up at times when you need a tanky item while having Jhin or Ezreal as your AD Carry, since you probably won't be building Zeke's with them.
Swiftie boots are the alternative to Mobility Boots, and can be just a straight up replacement if you don't think Mobility Boots are worth it.
Mikael's is extremely useful against high CC teams, and can be effective even as a first item against such teams.
Last but not least, Pink Wards should be purchased on every back, as playing for vision is essential for all supports.
Runes
As for Primaries, Aftershock is just too good to not take at the moment. This keystone lets you tank damage after go in for engages, and without it you'll be much, much squshier. Demolish, Bone Plating, and Unflinching are pretty default go-to for most thresh players, and I find them usually the most useful as well. Some other options to mention would be second wind and overgrowth. Second wind is usually really good into poke matchups where you won't get much value out of bone plating. Overgrowth is simply another option for Unflinching, if you are fine with trading tenacity in for some scaling health.
Running the stopwatch rune makes getting Gargoyle Stoneplate faster as you save 600 gold. However, the rune other than perfect timing can be up to preference. Most commonly used are Minion Dematerializers, but Biscuits can also be a decent option as well. Again, this is up to personal preference for the most part.
Assuming you don't want to grab a Gargoyle Stoneplate, then the page with precision secondaries is probably your best bet, as it gives you nice mana sustain, along with lots of tenacity.
Also, Mini-Runes can be adjusted depending on Matchups and Playstyles.
Finish
This guide should give you some insight on what items and runes to run, along with some basic matchup knowledge. However, playing Thresh is actually a lot more than just this, and it'll take time to get a feel for how people dodge, and how/when to use your abilities. For all learning Thresh, I'd recommend first learning when you want to Engage, or Disengage, as these are the primary functions of Thresh, who effectively is an engage and disengage tool. Getting a feel of your abilities and knowing when to fight and when not to fight are easily the most important things to learn about Thresh.
I would also like to mention that there really isn't a need to go for too many montage plays or flash combos. Those really aren't necessary to actually playing Thresh well, although they can definitely be cool if they actually happen in game.
