With just a doran ring and maybe mana pots if you need, can keep your mana up and keep the lane pressed as desired long enough to buy pretty much anything. I often wait until I can buy magic pen boots straight up and return to lane with teleport so I don't miss any exp.
Voidlings: If i plan to use malefic vision directly on a champion i try to wait for a voidling to be prepared to spawn to max out the potential dmg. It is all about the malefic vision, but if you have forced the enemy to recall and you have some tower time use your other spells besides malefic vision so voidlings will target the tower just as you do(if a minion or two are left malefic vision is worth the waste or to bring out a voidling if the other spells were not enough).
Doran's ring: this item is the best in the game for malzahar i swear. many games have be won over pen boots and 4 rings. stacking ap, hp, and mana restore passive. Some spell vamp and you have no reason to leave other than more items.
Masteries: if you notice there is some physical dmg and armor pen in the offense tree. both your auto atks and voidlings bonus from atk dmg and armor pen. the defenses are little to none, but if they cannot get to the other side of the lane to touch you, you're fine. thus far there is no garenteed counter meta against Malz, only a skilled chance to fight him, or you know, a five man tower dive. http://www.twitch.tv/buology/c/2800934
