At early levels you should focus on last hiting minions and soft trading with your opponent on lane. Vladimirs highest powerspike starts on level 9, thats when his Q (Transfusion) is fully maxed. In teamfights you should always start your combo with R (Hemoplague)all enemies that are affected with Hemoplague will take 10% increased damage, so your combo shoud look like this R > E-Flash > Q > W (Use W in the worst situations, it takes a lot of HP and has long cooldown) your R will pop up and heal you a lot after this one. Just like any other champions, focus squishy targets and grab your free win!

Here are some combos for Vladimir

E > Q
E > Flash (Hold E for a moment, then flash on your opponent)
E > Flash > Q > W (As I said, sometimes you wont need to press W, its based on situations)
R > E > Flash > Q > W (Once again hold your E before flashing, W - situational)

PHASE RUSH PROC

AA > Q > E (Cancel your AA animation with Q, after that instantly tap E to proc PHASE RUSH)
AA > E > Q (Tap E)
E > Q > W
