Being a threat.
Able to generate aggro (or forced aggro with a taunt).
Items, a tank with absolutely no tanky items will get focused down quickly.
Recommended that the tank is capable of initiating.

Tank: Someone that causes enough threat to make the enemies focus them (generating aggro), but also someone who can take the damage long enough for allies to make a difference.
Absolutely the only pure tank (defined strictly by being there to keep damage off allies) is Shen. That doesn't mean that he's the best. Most people won't target him at all on purpose anyway. And sadly, Shen has very little damage output. He is completely useless unless he tanks, and does it well.
Rammus on the other hand, can taunt and shield himself. He also has pretty good damage output and great backdooring skills. This makes him more versatile.
Galio has a great taunt, massive AoE that deals damage. He can also slow enemies while speeding up allies. I think by this point the rest of the tanks are just supports with high survivability.
I define support by basically any debuff (stun, slow, blind, fear, etc...).
Personal habitual activites? Utilizing gargantuan idioms to fabricate intelligence.

2. A non-ultimate snare (as a form of initiation)
3. Defensive-focused skills (block damage, or punish enemies for getting close)
4. Strong base-damage skills (gives the freedom to build for #1's needs)
Champions that fit the bill:

-Singed
-Blitzcrank
-Alistar
-Udyr
-Cho'Gath
-Maokai
-Galio (mage tank)
-Sion (mage tank, but his AD-skills make him a bad one)
-Swain (mage tank)
