When people start ganking more, get their first core item, have their ults, can take dragon (with less than 5 people) and uhm................. otherstuff.
Early game is over when a tower goes down.
Early game = lane phase
When the first turret falls, you enter mid game, and the team that downed a tower can start roaming and pushing to snowball to late game.
Mid game = when any outer turrets are still standing
If mid game lasts too long, late game will eventually be triggered by a massive team fight after tons of farming is done, allowing one team to push down multiple towers.
Late game doesn't occur at the same time for both teams.
Your team enters late game when your break all 3 outer turrets, thus giving you the potential to box in the enemy team's lanes, giving yourselves superior dragon / baron control.
When/If the enemy breaks your outer turrets and , the nerve-racking "better teamwork wins" late game will start.

I think this is the standard definition, however, in rare cases a tower goes down before 5 minutes. In these cases can we really say that we are in "mid" game if people are still pre-6 and still only have 2 or 3 items?
As for late game, I don't think it's necessarily defined by the number of towers up, since it is possible to win with one lane having all towers up. While this is more and more unlikely to happen as the game gets longer, it is still a possibility.

I beg to differ. Early game doesn't end when the first tower falls. I've been in games where the lanes were very equal in strength and the laning phase took forever. If anyone left their lane to gank, there would of been a consequence for their lane.
Personally, I think Early game ends when someone is forced out of lane to either help/gank or IS forced out of lane by gank/harassment/death/tower destroyed.
I mean I've seen strong laners take a tower easily by 4 minutes but tend to stay in lane to apply even more pressure.
Ya that too, I mean a fed mordekaiser is probably just going to keep pushing his lane, because he can, even if the outer turret is gone. Has early game really ended for him?
