I got mine with Amumu. I was tanking as Amumu and initiated the teamfight. Then the other team completely ignored me and went for all my other teammates. I was in the center of the fight and had 
Despair and Sunfire Aegis. I kept spamming Tantrum, did the clean up, and got my penta :D.

First pentakill as Tank Maokai. I was sent midlane because 2 people were AFK and I carried **** from there.
It was back at about 600 total games and then a more recent penta at about 1200 games with AP Nasus in an ARAM.

I can't remember if I had one. I believe I've once said that I got a penta, and have mentioned it to people when the subject came up, but now I just don't remember it. If I haven't it's quite a mystery with a full 2700+ games played o_o

My friend got an Ashe penta in like a 900 elo game though ... LOL
