Added a non gp10 build related to MTaurs findings. Keep in mind you will be sub CDR cap without reverie so will need to upgrade glacial to frozen heart if you care about the last 5%.
The final build was like what you have posted as Build 3 right now, except I took Heart of Gold at some point. I think I took Regrowth Pendant and one Health Potion to start, but Boots and several Health Potions would also be useful, especially if you want to lane agressively. I don't remember the details, but at some point my build was
I definitely needed the MP5 runes to make this work, but you get the holy trinity of HP5 synergies in exchange. Also, it's pointless to upgrade to Spirit Visage until after, say, Warmog's Armor. Other than that I think the order given there is fine.
EDIT: Oh, right. You need 9/0/21 to get 9% more CDR as it is (you could get another 1% from runes too if you really cared). Taking Heart of Gold and eventually Randuin's Omen caps the CDR just as well at the end as upgrading Glacial Shroud to Frozen Heart.
I tried a test run. Seemed to do ok for mana with Glyphs of Clarity (mp5/level) and no Philosopher's. I took Kindlegem instead and upgraded to Spirit Visage ( Warmog's Armor + Force of Nature is enough HP5 to make this worthwhile). This lets you rush the CDR a bit and you just about break even with the lost GP10 by saving the money you would've spent on Shurelya's Battlesong. Might not be worth the change if you really like the active, though.
Anyway, it's about as tanky as it gets either way.

As for the second question, I think you mean "if THEIR team has no AD carry, do you still get frozen heart?". The answer is no, you don't. Keep in mind that frozen heart is only included in a late dragging on game. Most of the time you stop at glacial shroud. You need that CDR, and we unfortunately have very few options. If we switch to CDR boots, we lose the tenacity so need to get that tenacity pendant. But we still need to get shurelya's so now we have a build with 2 philo stones... probably not the best way to go. So we stop at glacial to save cash and move on to other things.

I have changed the top build summaries for those that don't bother to read the actual guide. Changed to CV as the first has ghost. Ideally you don't take CV, but somebody has to and if your support won't step up, you need to.
Interesting thought about Randuin's Omen. You still get the Glacial Shroud for CDR, but if you get Randuin's, something else has to go. Warmog's, maybe, since Randuin's does the same (but not as well). Replacing Warmog's with Randuin's and not upgrading the Shroud probably will be all your gold for the game anyway.
I honestly might just try your build as-is with Flash and Exhaust, though. Three CCs + Exhaust is a lot, and Exhaust is better than Frozen Mallet procs anyway. I just know that when I'm botching skill shots, the mallet makes me feel a little better, but it's better to just not botch them.
EDIT: Never mind, I see you take both Heart of Gold and Glacial Shroud already, leaving it as a choice of which to upgrade. I need to learn how to count. I'm a big fan of Warden's Mail, actually, and it's redundant with Frozen Heart, which is probably the main reason I don't get Frozen Heart often.
