Basics
Lane Phase:
Ideally Garen support works best if engages happen as the first wave moves in. The silence allows for winning trades provided the ADC can follow up and get out. Taking damage is fairly inconsequential because of Garen's passive but do keep in mind that the more aggressive you are the more you are able to take advantage of the absolute ****ing confusion the enemy bot lane is experiencing upon seeing the beauty that is Garen support.
Mid Game:
Assuming Garen's support item is complete, his wave clear allows for effective split pushing provided enemies are focused on other lanes. Q can be used to silence burst champions and W to tank in team fights allowing Garen to soak most of the damage.
Items:
Trinity force - High damage and sheen passive is a great Q followup
Death's Dance - Crazy healing from ultimate that can be used to chunk enemy players mid fight to open up opportunities for the ADC
